Day 21 Progesterone.

I’m not sure what to do for the best any help would be much appreciated. I have endometriosis and have been trying unsuccessfully to get pregnant for 18 months. I have previously done a day 21 test and it came back normal. However I’ve moved docs and he wants it doing again.

My cycles are 27 days. That’s 12 days bleeding, 7 days until I ovulate then an 8 day luteal phase.

The doctor is telling me to do the day 21 test, 7 days before my period, which means just 1 day after I ovulate.

Alternatively, I have read if you know when you ovulate, which I do, you should have it done 7 days after you ovulate. Which for myself would be 1 day before my period.

Or should I just go in the middle, so 4 days after I ovulate?

This test seems floored and the doctors I’ve seen don’t seem to understand that this should change according to different women’s cycles. Before my laparoscopy at the beginning of the year, I had a 6 day luteal phase, so going on what my doctor is asking me to do now, I’d have been doing day 21 test the day before I even ovulated! Confused😖

What a mine field!

Would really appreciate some input on this

I think it’s likely you ovulate before you think you do.

How have you established your ovulation date stated above?

I go by ovulation sticks, basal body temperature, checking cervical mucus and I also have ovulation pain the day after my positive ovulation stick.

Do it on day 20 - 7 days before period is due.

Your short luteal phase might be contributing to your infertility so a blood test showing you don't ovulate at the right time would be useful in terms of getting yourself moved onto the next step in fertility tests.

Out of interest what day did you do it when your day 21 showed you ovulated? And what were your cycles like then?

These tests are for a referral so hopefully I’ll be starting the process now of getting some help.

I think last time I went for mid luteal phase and went for the 4 days after ovulating, 4 days before period. However work were awful for giving me time off so I think it was out by a day or so anyway but I can’t remember which way it swung whether it was closer to when I ovulated or closer to when I started my period. But I seem to remember it came out at 40? Which I think is well within normal limits.

Oh also, I always suspected my problem was the short luteal phase but when I paid to see a specialist for a consultation he was more interested in the prolonged periods and thought the issue was estrogen rather than progesterone?

I wouldn't worry too much if the day isn't right as a low result will be reviewed by a fertility expert and they can assess things properly. Good luck
