Well, it's hardly the 'fault' of 'someone in the editing suite'. They wouldn't choose the images. I think this is being rather overhyped. It was an image of an artwork (which you may or may not think is a good artwork) rather than a photo of Hindley. You may well think it was a mistake to include this particular image in the montage, but it has never caused controversy before and it was a tiny part of a very short film about London.

unintentional,i mean the intent to upset. the image has been defaced before at RA.so yes is known to be controversial, RA members resigned over it in protest
