AARGH!!! My sewing keeps on coming undone - what am i doing wrong?

Nametapes, Beaver badges etc etc keep on coming loose because my knotting off is obviously not working properly .

Can anyone explain how I should be doing it?

Ah c'mon, you know you want to post

I sew all the way round, then do an extra stitch without pulling it tight, then put the needle through the loop twice (making sure that if the loop is twisted, it still goes through twice, and not once one way then back out again ifswim). Pull it tight, then push the neeedle underneath the badge/name label, out the other side and snip the thread there (away from the knot).

Possibly you're snipping the thread too close to the knot?

I think you're right - I do exactly as you do, but then snip the thread right next to the knot - no wonder it's coming undone.
