Ruined washing - why? Please help!!!!

Every so often when I do a wash, some clothes come out with what looks like white bleach spots all over them!! And i don't use bleach - same detergent as always (past 5 years!), so have no idea what's causing this!
At first, was keen to blame it on dh but it's happening more and more and has ruined a new dress (worn once) and now my favourite pair of jeans (nearly crying with frustration as I have no idea what's causing it!). Not to mention lots of the children's clothes and other t-shirts...

Does anyone have any idea what I'm doing? Pleaaase?
Currently washing machine is on empty at 90.

what - permanent spots or just white spots that come out eventually?

If your washing machine drains into your sink outlet (most do) could it be cleaning products migrating across from your sink? Do you ever put bleach or drain cleaner down your sink drain?

DP recons it's because the washing machine is overloaded.

are you using washing powder at low temperatures (not colour)

if so they contain optical brighteners bleach and if nit dissolved properly may bleach?

what about using a bit less powder? half or even 1/4 of the amount is normally enough
