Putting up an unusual hook

Has anyone put up this Oliver Bonas shell hook successfully? It looks like you're supposed to screw something into the back (???) and we are baffled! Sorry to be such an idiot, but I just can't think where to ask.

Can you take a photo of the back of the real hook?

can you share an image of the back? Is it not you put a screw or nail in and then hook it securely over, does it have a hole in the back at the bottom?

Do you unscrew the shell, put a screw through the cylinder into the wall and then screw the shell back on?

Maybe the hook separates into two parts, and you screw the back one to the wall and then reassemble?...
