A serious question. Was the difference between them just age as well?
I've read about women in history who had dozens of them and can't imagine what that many found to do with themselves.
Can anyone help and/or suggest a useful book on the subject.

I think Wikipedia is your friend here. Look up "Lady-in-waiting" and "Maid of honour" for detailed descriptions and you'll also find some links at the bottom of the page.
