DH ankle chronically painful a year after sprain

Last summer DH fell from a height and sprained his ankle. They x rayed him at A&E and showed no breakage so he was on crutches for a week and then did the physio exercises they gave him. He kept it up for a couple of months but then a lot of very stressful life stuff came up which made him often skip physio, and he fell out of the practice of it, and by then he was capable of being on his feet all day so it was easy just to stop. A year on from the sprain he decided he should start doing physio again because he still felt unstable, but the exercises increased his pain. He went to the GP who put him on a waiting list to see a physio, and told him the ligaments may have healed poorly due to not keeping up with the physio, and his ankle may not go back to normal. DH said his goal was to be able to go running again and the GP gave him a look like that's very ambitious. We have no idea how long the wait for a physiotherapist will be but probably more than 6 months away. DH has been doing exercises the GP gave him and has kept it up twice a day for a couple of months now. While his stability and range of motion has improved, his pain is worse than before he re-started the exercises. The GP said it was fine to push through moderate pain to do exercises but DH is very dispirited that the pain isn't improving at all. He says he feels like he's dealing with a chronic pain condition now because he's in pain every day. And because he hasn't seen a specialist he has no idea what he should be doing and he doesn't know whether his ankle will be like this forever, it's depressing him. We can't afford to go private so we'll just be waiting several more months for an appointment. Has anyone been in this situation before?

An avulsion fracture can be missed on a normal xray, a ct scan could determine if there is one but obviously you would have to push for that.

Can't fluid/fibrotic scar tissue form? I'm trying to remember what it was called, but I know someone who had a knee sprain and presumably the same principles apply?
It might be a good idea to pay for a consultation with a private specialist if you can.

Im not an expert but I don't see how ligaments would heal "poorly" due to not keeping up with physio. Sounds like it is worth getting a second opinion/scan.

In terms of physio, has he tried putting his history and symptoms into an AI tool and asking it for physio? Any time Ive had NHS physio they just give you generic exercises that they print off the Internet!

Obviously a private physio would be ideal if you can afford it.

Has he had any hands on physio? Exercises alone may not be enough especially if it has tightened up over time. A sports physio might be worth the fee.

I agree he needs better imaging, ct scan or MRI. I went through similar and turned out I’d completely torn a tendon in my ankle and it needed surgery to put it together.

I think he needs to push for an MRI to see exactly what the damage is to his ankle. Then the physio will know what they dealing with when he does have an appointment.

@SwizzlesTwizzles does it hurt to touch as well, or just painful when he moves it? Is it painful to put socks on?

Have you looked at Complex Regional Pain Syndrome?

I would definitely take this further for more investigation. My DS tripped a few months back and was in a lot of pain so we took him to A & E. They diagnosed a sprain but the following day we thought something wasn't right so we took him to a different hospital. Anyway to cut a long story short, after months of waiting for tests and appointments, it turns out he had torn his ACL, his LCL and damaged his common peroneal nerve in his knee. He has just had surgery but it's looking like the the nerve cannot be repaired. Mis diagnoses are far too common and they have far reaching consequences.

That was my first thought. My eldest has it since she sprained her ankle when she was about 12
