Anal fissure still painful ten weeks after Botox and pile treatment

Hi all,

I have been suffering from an anal fissure and piles for the last 5 months. Approx. ten weeks ago I had a botox injection for the fissure, and rubber band ligation for internal piles. I have also had quite a few external piles flare-ups in the last few months.

I am still experiencing pain following a bowel movement, but not after every bowel movement. At the moment, I have one bowel movement per day, sometimes two, in the morning after waking up. I can have two or three days without feeling pain following a bowel movement, but then for no apparent reason there's pain after a BM. I have followed the advice given by my GP and consultant. I've been eating plenty of fibre, drinking approx. two litres of water daily, stool softeners, taking sitz baths, using ice packs, adopting the squatting technique, using a hand-held bidet.

I had a follow-up appointment with the consultant 6 weeks after the botox injection. He thinks there's no problem because the sphincter is relaxed and that I'm doing fine. I tried telling him that I don't feel fine. He said that I can't get another botox injection for 6 months, and that he doesn't perform surgery because of the side effects. Anyway, it wasn't really clear if he's going to do another botox injection or not. That's why I hate dealing with GPs and consultants. Each appointment is feels rushed, and I end up coming away feeling not listened to.

The botox is due to wear off in a couple of weeks time. I'm extremely worried about what will happen after this. I thought at this stage everything would be healed. Is it normal for a fissure not to be fully healed ten weeks after getting botox? Will this fissure ever heal?

Can you see a different consultant?

I had multiple fissures from childbirth and suffered for months. I was prescribed a cream (sorry I can't remember name. It may have been Diltiazem but I'm not 100% sure) but it is usually used for heart issues I believe as it increases blood flow and relaxs the area. It also doesn't cause the headaches like with the GP prescribed cream.

My pain went away fairly quickly and I was healed within 6 weeks, and they've never came back 🤞

I remember my consultant saying it isn't clinically authorised for fissures but it can be used for them when directed by a consultant. Could you ask your consultant about this cream?

I had two rounds of Botox and it didn’t work. I get regular horrendous flare ups but can’t face the op. Good luck 💐

Mine didnt go after 2 rounds of botox, I had to have a advancement flap - it cured me and I just wish I’d had it years before I was finally allowed.

Have they really looked at it? If it’s not a fissure but a fistula, you would need surgery.
