I Have recently had a baby, midwife yesterday confirmed that i have an infection in the tear but with 2 children, neither of which walking and only a single buggy i dont know how to get out of the house to get to the doctors for anti-biotics. Im really desperate to get it sorted as i have been very uncomfortable since monday.
Do you think a doctor would make a house call?

I'd ask dr to order antibiotics and leave them at pharmacy for you, ours do this (i had some when i got mastitis)

Can your midwife or HV contact your DR and ask for a prescription?

Your mw could probably call the doc and get him to make up the prescription, then you (or someone else) could collect it later.
