Has anyone been through the CHC process for a young adult?

Just trying to prepare ourselves, DS has a progressive neurological condition similar to MND, he went through the first assessment, just wondering what the next stage looks like.

Is there an actual assessment with him or is it based on reports?

The next stage will be the DST (decision screening tool) meeting with a CHC nurse assessor, a social worker, you, your son whereby they’ll look at the domains in more detail and make a recommendation. It will then go to a ratification panel if the recommendation is CHC funding.

It’s the nurse’s job to research all the info but go along prepared as they often score lower than it should be if they can get away with it!

I have looked up the further 4 classifications, does that mean we basically need to explain how he is affected in each of 12 x 4 sub-categories? And I expect all needs to be supported by reports, almost certainly coming from different places?
