Error on patient record. WWYD?

If you had an error on your patient record, 1) what would you do about it and 2) if you notified them what response would you expect from your GP?
The note "Non-organic psychosis in remission" keeps appearing in my online record at random intervals, not related to any contact I've had with the surgery. I have no history of psychosis nor substance misuse.

My mum had a letter listing she had heart failure. She didn't it was an error when I questioned it.

My surgery had me down as a ‘heavy smoker’ and kept sending me info about clinics to help me stop smoking. I had to email them and ask that it was taken out, I’ve never smoked and have multiple mentions on visits for resp infections “doesn’t smoke”.

They didn’t really respond, just took it out.
