App to record health records

I'm looking for a way to record symptoms, test results, appointments (can never remember last smear test etc).

I've had some recent health concerns and dealing with multiple GPs who don't seem to flick back my records to compare notes, so all this is reliant on my memory.

Does such a thing exist? Not in England, so the NHS app doesn't work for me.

I'm 66 and have a host of long term conditions plus a history of breast cancer.
I document everything on a google doc. After every contact with a doctor or nurse I write a note of what was said and the plan. I ask for copies of all letters from consultants and keep those.
I also keep a record of symptoms which might come and go in between appointments.
A lot of this is on the NHS app but not all. It's also interesting to compare what the HCP has written about an appointment compared with what I have noted.

It's not as good as an app but I can organise it to suit and it's invaluable when managing long term conditions. So for example I might only see my rheumatologist every 18 months but I can check my notes and see what was discussed at previous appointment.

Thanks Poffy. I'm looking for something easier than a spreadsheet but this may be best option.

Have you looked at something like WomanLog? It's essentially a period tracker, but it has a huge range of symptoms you can select against a date, and also the ability to add notes. It has peri and meno settings too.
