Any thoughts on these symptoms (old man, bad heart)

Previously super fit and healthy 60 year old man.
Diagnosis of moderate aortic stenosis.
Also SVT on bisoprolol. Has a pace maker fitted as heart previously stopped. All of this is within the last 12 months.

The issue is , he keeps having episodes of 'confusion' - he describes this as having thoughts from different times all intruding at once, not sure how long it lasts and feels confused, but is aware he is confused. At the same time he will feel chest pain and sick to his stomach / dizzy, if he is standing up he has to lie down, sometimes this has been on the pavement etc, but nothing shows on heart monitor during these episodes.
additionally, perhaps related, he has pain in the night through his back up to his shoulder , this has been investigated by ultrasound but nothing found. On bad days this pain stops him sleeping and he has no appetite.

oh wise mumsnetters, maybe you know what is wrong

When did he last have an echocardiogram and ECG?
Does he have indigestion/reflux symptoms?
Any family history of heart/stomach problems ?

You're absolutely right, except that he's morphed into an elderly man overnight !

ECG this week, echo cardiogram last month to check the aortic valve. Not seen anyone as follow up. Scheduled for ablation for the svt.

When did he last have his blood pressure checked, could be it’s falling when he stands up making him dizzy, dizziness can make you feel sick.
The episodes of confusion may be something unrelated to his heart issues. I think a gp appointment is needed / easier said than done!!

Either still having episodes of SVT or having symptoms from his aortic stenosis or both. Plus some anxiety.

Thanks, I don't think it is svt as he has an apple watch and it didn't report high heart rate during these strange episodes. Could be aortic stenosis I suppose. He has not see a cardiologist in person at any point.
He has recently seen the GP who was concerned about his pancreas, but considering the length of time this has gone on for, it strikes me as hopefully unlikely.
I considered maybe a blood sugar issue but this was checked during his most recent episode.
I honestly don't think anxiety - GP suggested panic attacks , he is just not someone who is ever worried about anything, so I wouldn't like it to be explained as that if it could be anything else.

It's hard isn't it when you feel no one is able to put everything together.
