Social History or Self Harm depending on context.

(God only knows /Typical for Swindon / Inbreeding Evidence here….)
All seen on patients notes in the early 70’s.

FH = Family history e.g. history of medical disorders in direct relatives e.g. “mother had Stroke aged 55”
PMH = Past medical history e.g. a list of your medical complaints e.g. hypertension, type 2 diabetes
SH = Social history e.g lives with partner or lives alone etc

I think most most people will be able to work out that insult.

Now adays yes, but back in the day without text talk and normal use of acronyms in everyday life I don’t think people did
