All seen on patients notes in the early 70’s.

FH = Family history e.g. history of medical disorders in direct relatives e.g. “mother had Stroke aged 55”
PMH = Past medical history e.g. a list of your medical complaints e.g. hypertension, type 2 diabetes
SH = Social history e.g lives with partner or lives alone etc

Now adays yes, but back in the day without text talk and normal use of acronyms in everyday life I don’t think people did
