Any thoughts what might be going on now? (Foot injury)

Background:

[May or may not be related, C. 3 years ago I fell off a ladder onto foot and heard a crunch. No Xray but painkillers and crutches given. Very painful and swollen but seemed to heal in a few weeks].

About 2 years ago, extreme pain in achilles tendon. Treated for tendinitis - NSAIDS, ice, physio. Didn't heal for over a year. MRI showed inflammation in a different tendon but not achilles (pain clearly achilles not the posterior tibial tendon, where RI showed inflammation. After more anti-inflammatories, further physio, got mostly better. Able to walk and start to job again, but always with a feeling of being aware of the injury, though not exactly painful.

Over past week weeks - really odd, slightly sickening feeling of a gap inside my foot sometimes when walking. Sometimes suddenly really painful, but after a few steps, rights itself. Not in either achilles or posterior tendon area, more kind of mid-ankle/foot, outer side.

I don't know if I'm being just a bit paranoid due to the various issues with this foot, but it just doesn't feel right. I'm not sure where I should be ding or looking for. Any ideas, please?

If you have a good physio I'd go back again - feet are important!
