Any podiatrists? Toe/nail issue WITH PICTURE

Excuse sock fluff stuck to it & down my nail, I wear black socks in work.

I assumed several months ago that I must have a fungal nail infection or something as this one toenail periodically hurts and appears thicker underneath. I tried to scrape under my nail with the edge of tweezers to clean and it was agony as the white fleshy bit is attached/sensitive.
I have religiously applied Daktarin 2% clotrimozole to it daily for several months now, scraping it down under the nail, but there is no change. I thought a little while ago that it would just take some time to grow all the way up -even though it consistently looks like just the top 1/3 of the nail is affected-it just won’t go. It is painful, as if when it’s growing it’s pulling the fleshy thick bit off the nail bed iyswim.

What is this? How do I get rid of it? It seems such a minor issue to bother the GP with but it’s painful.

Looks like a fungal nail infection. There are OTC treatments you can buy where you have to file away the nail surface and apply the treatment regularly for some weeks. It worked for dh. I'm too squeamish to file that much, so I tried the paint-on stuff which doesn't require filling, but it didn't work.

I initially thought Fungal but it’s not the usual thickening of the nail you see with fungal, because it’s soft and fleshy and attached to the nail bed, rather than just a thick nail.
