So itchy

For the past three days my shoulder has been absolutely unbearably itchy. There is no rash or nothing there. No dry skin at all. Now bruises have started to appease from where I've scratched. I haven't broken the skin. Should I be worried about this?

I've tried an antihistamine but it's not making much difference. The itching at night just ok my shoulder is unbearable :(
