Swollen toe (picture included)

My husband has had a very painful, swollen (middle) toe for months now. It's not red or hot, but visibly swollen and tender. His toes are thin and bony but this one is almost shiny as the skin is so stretched, though the photo doesn't really show this. The pain comes from the base of his foot, and spreads down the one toe.

Last year he went to the GP about it, who couldn't work out the problem, though we ruled out diabetes, gout and arthritis. The pain went away to some degree for a while but is now back with a vengeance. Ice, elevation and ibuprofen gel are providing some comfort but it's not going away.

Does anyone have any ideas what it could be? Thanks.

Morton's neuroma? I've had similar and awaiting an ultrasound to confirm diagnosis

Sprain? Fracture? Does he do a lot of running?

I would go back to the Gp with that. It has gone on too long.
