Nappies smell like bread dough: thrush despite Nystatin?

Baby (now 7 weeks) has been on Nystatin 4x a day for 4 days now. Nappies still smell like bread dough. Her bum looks better, she's never had the oral white plaques, but I'm getting bothered that the apparent gut infection isn't getting better.

My GP said she hadn't heard of fluconazole being used for treating infants, which I know isn't true.

Anyone with any advice please?
