External/internal piles - worth getting removed? ?

I'm really anxious about anything going near my arse at the best of times and, having had some antibiotics recently, it is definitely not a happy place. I have 2 external piles that bother me and (I suspect) several internal. I became constipated straight after having antibiotic diarrhoea and now the whole area is sore and feels heavy. I even have some discomfort on one side when I sit down, that even makes my vagina feel sore. I assume it's from an internal pile or something (have a vague memory of having it post pregnancy which is when all this pile business started). I suspect I may have some kind of prolapse as well but I'm not sure about this.
I'm happy to do pelvic exercises to help this resolve but with the piles, particularly the external ones is it worth getting them removed? I've heard some awful stories of people having a terrible time on the loo afterwards and as someone who struggled with bowel issues as a child it's something I'm quite anxious about.
Any positive experiences or is it not worth doing? Thank you.
