Chronic paronychia anyone?

Hi I have had a paronychia on one of my big toes since December. It’s been really painful and GP not much use. Tried soaking etc. Eventually pus coming out of cuticle area so given 7 days antibiotics. It didn’t clear up and so now I’m on a 14 day course and my other big toe is also showing signs of getting it despite being on antibiotics, it was so painful in the night last night and if it gets touched.

It’s not like a finger one where you can see a pus filled bit it’s swollen and red and clearly has fluid on the base of the toe.

Any ideas? As antibiotics clearly not helping.

Been to a foot place who checked nails etc and no nail issues. I suspect caused by having poor circulation in my feet over winter. Thanks

Bump. Have same issues with fingernails. Dr prescribed terbafine back in feb but I still haven’t taken it. It comes and goes

Is that anti fungal? Is it a cream or a tablet.
I’ve now been told to soak in vinegar each day and apply an antibiotic cream. GP is clueless on the situation! Really painful
