See a good physiotherapist, not a chiropractor or osteopath.

Also the APPO dvd Pilates for lower back pain is a godsend.

I suffered for 5 years until I saw a physiotherapist.

I had 7 months of pain and in July went to a chiropractor. Wasn't sure what to expect. Its been amazing. I'm pain free and gradually building up my fitness again. I have significantly reduced PMT and the tension in my neck and shoulders is muchly reduced. I didnt realise how much tension was there until it was released.
