Charities for post surgery care

Does anyone know of any charities that offer support for people recovering from transplant surgery after they leave hospital? A distant relative is due to be released from hospital at the end of the week. He lives completely alone and has no one that can take care of him. Is there anyone who offers help in this situation either for free or at a low cost?

He shouldn’t be released until he can care for himself satisfactorily. They may need to do an assessment for a care package. It’s terrible state to be in and we shouldn’t have to rely on charities to access care. What organ was transplanted as will be easier to think about potential charities? Alternatively he could employ private care. When we explored this for my dad was about £25 per hour.

@JoanieCash he’d be fine to go home if he had someone with him tbh but it’s just that he lives alone and all his family members for one reason or another couldn’t go stay with him. It was a kidney transplant.
