Knot on arm 5 months after injection

5 months ago I was in the ER, I have chronic gastroparesis. I was seen for excessive vomiting and received a phenergan injection in my arm. I’ve had a hard knot every since. It only hurts when I touch it and feels like a fresh injection. Can anyone tell me why?
