Your survey would benefit from an information page prefacing it, outlining aims etc. & contact details of supervisors - this is the norm. The fact it just launches in with no explanation could be a bit off putting and not incline people to fill out I’m afraid.

Also, I might not understand what your definition of a ‘fruit based snack’ is - a definition & examples would be useful. When you say ‘space’ do you mean size? Storage at home?

If you haven’t already done so, ask someone unrelated to your project team to read through & check for understanding.

I think it is fairly clear that this survey has been done by a younger user. Even more clear is the fact that the user means space as in outer space. Who would create a snack based on size or storage at home?
