Is this pyelonephritis? (Kidney infection?)

Hello, just looking for some advice/experiences.

I’ve got a UTI at the moment that’s been treated with a 5 day course of antibiotics. I’ve just had my 5th day dose. The UTI symptoms have persisted (the burning especially 🥺) and early hours of this morning I was woken up with awful mid-lower back pain, mostly on my left side, and it slightly radiates round to my tummy. I hardly slept last night and I’m that’s currently soothing the painful area with a hot water bottle.

I know that back pain is a sign that the infection has spread to your kidneys but I’ve checked my temperature and I don’t have a fever, and I’ve read that a kidney infection almost always is accompanied by a fever. I am feeling nauseous but I think that’s more associated with the back pain than anything else. I’m due on my period in the next day or two so I’m wondering if it’s that rather than a kidney infection but I usually get only very minor cramping with my period, nothing like this.

Has anyone had a kidney infection without a fever or do you think I should just put it down to extra bad period cramps this month?
