What's this spot?

I've had this hard "spot" for about 12 months now. It definitely wouldn't squeeze, and is sometimes red but never sore.
It's on my chest near my bra strap but not irritated by it.
Do I need to do anything about it?

Yes. Any unusual spots they aren't normal acne and you've had for that long should get checked. You're obviously worried about it, please do get it looked at

From the black dot in the middle it could just be a large blackhead but still something that can be easily sorted
