toe pain advice

Hi,
I always suffer with very cold feet but recently when my feet have warmed up (after a bath, central heating on, extra sock etc) that my big toe becomes painful - like being stabbed.

If I put a freezer pack on to cool my foot down it will subside. There's no redness, marks and its not the toe itself being hot - it's just a general warm up of feet.

It's not sore to walk on - probably more noticeable when just sitting.

I've not been able to see a doctor (how our practice works) and eventually got to see a nurse who told me to rub ibroprofen cream in for 2 weeks. This doesn't seem to really do anything.

Any ideas?

Thanks - I think that was what the nurse was getting at as she kept asking about it being red and swollen but it isn't. It just looks normal - no change at all.
