Help/advice needed

I really need some advice as I feel like nothing is working. I believe my baby has reflux
Only sometimes he brings milk up but constantly hiccups after feed, has a cough, sneezes, fights the bottle, arches his back in pain during and after feeds, hates being on his back and only settles at half an hour a time on his back in the day. He is better at night but after the second night feed we are back to square one. I can hear the milk pooling in his mouth and his tongue always has milk on it. He thrashes around when he is on his back and makes grunting noises. He also gags and sometimes chocks on the bottle which is very upsetting. He also seems to be getting worse not better!
Things we have tried
Gaviscon ( seemed to help a little but still arched his back in pain)
Now on Ranitidine 3x a day (was initially on lower dose but this was increased last week have not seen any change in his symptoms yet) and also carobel to thicken the feed.
He is fed 180ml and I add one scoop leave for 15 mins but still seems to bring up milk any suggestions on how long to leave and amount to use?
this is my first baby and I have been feeling really down and am finding this all very upsetting. I don't know what to do to help alleviate my babys pain and discomfort. I am hardly sleeping and cannot get anything done as he has to be on me constantly in the day as he does not settle.
I did not want to put him on the ranitidine but feel like I do not have a choice as he is very distressed so am trying to do whatever I can to relive his discomfort.
How long should he be on the ranitidine before I see an improvement? as I do not want him to be on the meds if they are not helping at all.
Would really appreciate some advice thank you
