I’m very worried about my friend. They are very over weight, smoke and drink too much and take no exercise. I am not judging them I am just really very worried they will not live to see their children grow up. How do I encourage them to make positive changes without sounding like a bore, making them feel crap, pushing them to eat/drink/smoke more.

You can't. It's a cliché, but they have to want to do it themselves. And they already know they don't have the best of habits.

I guess you could suggest going out on regular walks or yoga classes together or something (or train for something like a moon walk) but otherwise just be a good friend in other ways; listen, compliment them about their good points, boost their esteem.
