I just walked into the most local practise and asked for registration forms.

look on Primary Care Trust website, upmystreet, ask for a recommendation. I asked a few local friends then took our NHS cards into the surgery.

Same here.

You can go to Up My Street and enter your postcode and it will tell you all the surgeries you're eligible to sign on with - sometimes you're in more than one 'catchment' and can choose, if they are taking new patients.

The local pharmacy, library and post office should have a list of local surgeries. Do a bit of homework first about their appointment systems and surgery times before you register to make sure you find a system that suits your needs.
