Help with sick note please

I had an urgent appointment with GP today. I have been off on annual leave for a week and due back in work tomorrow. I've not felt well, and got an appointment for today.
She has signed me off work. The sick note says for a week from 27/9 to 4/10.
Do I return to work on the 4th or the 5th?
And do I get a days annual leave refunded for today 27th?
Thanks in advance for help, I've not had one before and didn't think to ask before I left the dr.

The 27th would go down as sick not holiday and you return to work on the 5th Oct 🙂
