How do you reduce insomnia?

Insomnia back with avengance. Never suffered pre kids at all. Now it's pretty bad. Struggle getting to sleep and often wake and then can't sleep again. Roughly 3-5 hours a night.
Any success stories out there?
Things that help me - bath, reading book, purposely not thinking about my worries, no iphone after 8pm, exercise in day, clean, calm bedroom.

Have you tried meditation? I downloaded that headspace app and often use it before I go to bed to help me unwind. I used to sleep really badly too. It would take me ages to fall asleep and couldn't switch off my thoughts.
