Help diagnose me, TMI

Fed up with some health issues that have been plaguing me for years. Wondered if anyone could give me some advice, or help diagnose me ha. It's all a bit TMI.

The worst problem is chronic anal fissures, I first started suffering when the doctor prescribed me fybogel for my "ibs." This caused me to have huge bowel movements, and I tore quite deep. After that I was in pain for about a year.
I tried all of the creams, but eventually had botox twice, and a procedure where they cut away the scar tissue. I'm not sure if the botox helped much, I think it was them removing the scarring that helped it heal.
No pain for about a year, but now for the last 6 months I keep getting 2 fissures that continually heal and tear. I always have loose stool yet I always seem to tear and I don't know why :( I have a skin tag that swells up when it happens too.
I've also had problems with the skin at the bottom of my vagina tearing when I have sex.

  • I had a blood test for coeliac and crohns, but they said it was normal, I don't know how reliable bloods are for these illnesses though.

I also have a rectocele(diagnosed by surgeon), and I can feel the bulge inside me. This is also weird as I haven't had children yet, not overweight, and I don't really suffer with constipation, it's usually diarrhea. Yet sometimes the diarrhea is difficult to pass, I think this is a combo of the rectocele and tight pelvic floor muscles.

I usually go to the loo in the morning, and after I go my abdomen is really distended, and my gut just feels sore and inflamed. It will gradually go down, and then bloat up again mid afternoon to early evening. I have sore spots in the left side of my abdomen, which sometimes feel lumpy.

I often feel tired, and sometimes I will have a weird drugged up feeling after my lunch and will have to go and have a nap or it's really hard to keep my eyes open.

I've tried exclusion diets in the past but it doesn't seem to make that much difference. I've tried the fodmaps diet but this is really difficult to stick to and I lost too much weight. Again still had the fissures.

I'm waiting on an appt to see another colorectal surgeon, but I don't want the surgery where they cut the muscle (LIS) as I'm scared about incontinence issues as I get older/have children, especially as I'm having problems with a prolapse already :(

I don't know what to do now, feel like I've exhausted my options, it's a really horrible thing to live with. Anyone have any suggestions?

Forgot to add, I'm 29, all of this has been going on since I was about 23/24.
