Tick box consent forms

AIBU to think consent forms in hospital shouldn't just be ticks to say what you're consenting to, then you just sign it? What's to stop them ticking boxes after you've signed the form?
The form I signed had tick boxes for General Anaesthetic / Local Anaesthetic / Sedation. I specifically only consented to Local, even though they were doing their damnest to get me to agree to a GA, then sedation.
Shouldn't there be boxes to tick 'no' for when a patient specifically dissents to something?

Your refusal to consent to GA will be recorded in your notes.

You cannot be tricked into a GA. You'd notice.

Surgeons/anaesthetists don't tend to go round foisting GAs on people who don't agree to them.
