Mole or spot or what?

Partners back of neck. Any advice on what this is?
He said he had a spot that's bothering him and can I look. We aren't sure if he had anything there before and it's grown or if it's something new.
He said it hurts if he pokes it. The tops a bit scratched where he's been fiddling.
