9 week old with bad cramps - trouble pooing

My 9 week old really strains and struggles to poo. It goes on for hours and he cries and cries. When he finally "goes" he's calm for a bit, then he feeds and it starts all over again! Have tried all over the counter meds but they all seems to bring wind up, not force it down - doc gave me med to make the poo softer but that's not the issue: it's soft when he finally goes!
It's really affecting his sleep (and mine!) he wakes up at 3 and is in pain until he manages to poo at about 7.
Help!
(He is breastfed)

Did the doctor mention testing for lactose intolerance? I would go back and see him/her again and explain.

If the meds are bringing his wind up - that may actually do some good as the wind is not going into hi s bowel in the first place. It may take a few days to notice the effect of this though.
