How do I go about seeing my medical notes?

Just that really. What is the procedure? Do I have to pay? I particularly would like to see my notes around the birth of my child as experienced PTSD afterwards and just wondered if anything could've been done differently....clutching at straws really and am mostly over it all now but feel that to read my medical notes would be the last part of me recovering (child is nearly 5 for goodness' sake!)

I saw mine following the birth of dd2. I had the option to go back to the hospital and go through them with the consultant midwife, or to pay to have them photocopied and sent to me. I went through PALS to arrange it. If you look on the hospital website there should be a link to PALS (patient liason) I should think.
