Doesn't anyone know if and how you can request a copy of your hospital notes following an operation?

Try direct or through GP or referral. You might need to pay an admin fee

Contact communication dept or Pals at the hospital where you were treated. They should have the access application forms. Any particular reason you want them? Often if you're unhappy with treatment etc. it doesn't clarify anything due to the medical terminology.

Pals = Patient advocacy and liaison service.
