a question about health records

Does anyone know whether a request for health records would include everything health related including GP visits, hospital appointments, operation details?

If you request medical records from a GP you get what the GP holds so would include any visits to GP, correspondence from hospital etc. If you request from hospital, just their notes/correspondence etc.
