Sounds a bit like milk spots to me. Too much dairy produce seems to cause it in susceptible people - often on upper arms?

Sorry, now reread and you did say. A couple of children I looked after, dp, and now ds get these if they have had too much dairy produce. They look like tiny little whiteheads and are squeezable. Come in patches of a few inches across, most usual place the arms so maybe this is something completely different.

Could it be a heat rash or sort of acne triggered by sweat at contact points with clothes (has she worn a rucksack for example).

The ones I am thinking of are different to the ones in the photo, more like loads of little raised bumps, some with a white head. We always just called them milk spots because we reckon there's a link with dairy intake. Perhaps should say 'milk rash' instead.
