Ok, I was a bit rubbish when I was pregnant. My GP recetionist made a mistake with my exemption card form so they wrote to me wanting clarification and I never followed it up.....
But now DS is 9 months old and I need some of that lovely free dental treatment before he turns one! Anyone know how I get a card now? what form / where I get it from?
