App for health records

I'm looking for a way to record symptoms, test results, appointments (can never remember last smear test etc).

I've had some recent health concerns and dealing with multiple GPs who don't seem to flick back my records to compare notes, so all this is reliant on my memory.

Does such a thing exist? Not in England, so the NHS app doesn't work for me.

Does your GPs have an app, something like Patient Access or AirMid UK? It would be linked to the system they use.

With mine I can see all previous appointments and what's been recorded by them. Can also look up my test results through it.

No as far as I'm aware this doesn't exist via my GP.

My GP can't even see a history of my screening appointments, so for example, I go with a breast lump and they can't see a history of my breast screening appointment or bowel screening "unless there's been an issue with it". So backward.

I keep all my details in an Excel spreadsheet - if you 'PM' me your email I'll send you a blank one.
