Looks like a monstera, those are air roots. They're ate climbers in their natural habitat so it's normal. Might need a bigger pot though

Quick google search says they like lots of drainage and not overly wet soil. Probably needs a bigger pot too

You could repot to a bigger pot and pop a stake in, a mossy one is easy to climb, and train it up there to stop the weight of the aerial roots bending it like that.

Yes it’s a monstera / Swiss cheese plant. Will look at repotting and generally taking more care of it!

It doesn't look unhealthy -- just unbalanced! They are quite robust, and the aerial roots will be happy enough out of the soil. But a bigger pot will make the plant more stable - and I suspect it would enjoy the extra food.
