Could I use the spread neat to cover a cake or would I have to make it into a buttercream?

Personally I'd find it too rich/sweet to use neat especially if covering a whole cake. I'd make into a buttercream

sounds truly inedible

I think it would be quite tricky to use to cover a cake and very rich. You could probably use it as a filling between layers but I'd definitely mix it with frosting for the outside.

Absolutely not! It would be sticky, sickly and unattractive.

Biscoff buttercream is utterly heavenly:

125 g butter (soft and at room temperature)
250 g Biscoff spread (smooth)
185 g sifted icing sugar
A little milk

Beat Biscoff and butter together until blended. Add icing sugar and spoonful or two at a time, beating well between additions. When all the sugar has been beaten in, beat milk in a spoonful at a time until the buttercream is almost like a dense mousse.
