Caramel Suprise - recipe to create at home

I love a shop caramel suprise from Waitrose/Sainsburys but would like to try a recreate at home.

Any idea on how to make the caramel bit?

I would make custard without sugar and stir in dulce de leche or nestle tinned caramel
