Who decides if home care is failing ?

My elderly neighbour has been in hospital after a fall for thr last few months. She has no immediate family but was adamant that she wanted to come home so the hospital has put what I think is a very limited care plan in place for her . 40 min morning and evening. It's only been a week and I think it's clear that this is not going to work .. she's simply can't walk anymore .. she's still adamant she will improve though. Who calls it or do we need to just let the shit hit the fan. ( I think she's seriously hoping that neighbours etc will step in - she mentioned a rota) which we simply can't do. All have kids and jobs etc but it's hard to walk out the door knowing she won't be able to feed herself or walk to bathroom..
public health nurse is due tomorrow. Can she say this isn't working?

I think (in the UK) I’d call the local Single Point of Access and report your concerns, and keep doing it.

If your neighbour asks for help that you’re not willing or able to give, suggest to them that you call an ambulance?

You're a good neighbour to be this worried, and you don't have to wait for it to fall apart. Ring her council's adult social care team (or the Single Point of Access, if that's how your area works) and say plainly that since coming home she can't walk, feed herself or get to the toilet, and two 40 minute calls aren't keeping her safe. Ask for an urgent review of her care package, then put the same thing in an email so there's a record. Tell the nurse tomorrow exactly what you've seen and ask her to raise it with the GP and the council too. It's her right to choose to stay at home, so the aim is getting the right support around that choice. And please don't feel you have to fill the gaps yourselves. (Full disclosure, I co-founded a site called My Lighthouse that matches families with local carers, but I've many years experience in care and this is just general advice.)
