Age 80 frailty test

My aunt recently turned 80 and says she received a form / frailty test, asking questions about her mobility and activity. She says it also included an option to add DNR to her medical records, which she did.

Does anyone know any more about this?

Who did she receive it from? What were the circumstances?

Not sure, it was a brief conversation which started with her saying she wouldn’t want to be resuscitated, and she mentioned this form which I assume was from the NHS.
