Grandad in rehab hosp. What next?

Hello. Moved to this thread as I didn't know it was here..

Hello.
My grandad had a stroke in Nov. I'm his next of kin and I am trying to learn things quickly! He is coming out of hospital in 6 weeks. We are hoping he can go home with a live in carer. Does anyone know what he is entitled to for support? Someone mentioned he could get things out in his house to help him get around? Has anyone been successful with the Attendance Allowance? Any advice would be greatly appreciated. I am up every night trying to research everything! At the moment we thing he doesn't have capacity to make decisions. I think he has dementia. We are not POA. He didn't make one. I am applying to become Deputy but the solicitor said this can take 12 months!

There should be physios and occupational therapists at the unit who can advise. Before discharge the OT will meet nok and home visit to assess any equipment requirements (grab handles, perching seat, toilet aids, trolley etc) , depending on progress. There should then be a community care team to visit for personal care, meal preparation etc and reassess his needs once home.

Des he have a social worker? Attendance allowance is not too tricky- but order postal forms which will be dated so claim will be backdated to date stamp.
Get help from cab/ age uk to complete to get the right word,s.
