Retrospective CHC Funding

Is anyone able to tell me how to apply for this? My dad was awarded full funding (for his private care home) under the fast-track system, dated from when they received our application (at the end of July). We were amazed, but grateful.
However, he actually came out of hospital 6 weeks prior to that, with the exact same needs. The only form I've seen online is the same one we've already filled in, but with "date of death (if applicable)" on. He has now in fact died (last week), and whilst we're running on adrenalin sorting out paperwork etc, we wondered if we should get going sooner rather than later on applying.
What steps should we take? Is it wrong to apply?

He was already in the care home prior to being admitted to hospital (Spinal Cord Compression from metastatic bone cancer) and was discharged back to their care without any consultation with us. They spoke to the care home, realised he was self-funding and probably threw their hats in the air, as otherwise he could not have left hospital.

Just ring up the local CCG to discuss. You can apply to make a retrospective funding claim , if you believe evidence supports it. IME they will not go beyond 12 weeks for fast track and only if evidence supports that it could have applied before it was agreed. But they likely have a whole procedure / policy on this.
